Insurance Accepted At Sober Stages

Sober Stages accepts insurance from a wide variety of providers, but you’ll still need to verify that your specific plan will pay for your treatment and determine which treatment services your policy covers. Don’t worry though– it sounds complicated but, our online insurance verification is actually a very straightforward process.

In any case, not every insurance plan covers the same courses of treatment. Our team members and ou health insurance verification system are able to act as a preliminary estimation of your in-network and out-of-network coverage quickly and easily. Since we know how complicated insurance policies are, we have put together some information to help quickly define some of the terminologies you should know so you are aware of your health care plan and benefits.

Deductible

The amount you are required to pay for covered health care services within a plan year before your can expect your insurance to pay for them. For example, if you have a $2,000 deductible, you pay for the first $2,000 of covered services yourself (out of pocket), and once you have spent $2,000 on those health care services, the only costs you will expect to pay are for coinsurance and copayments — your plan should cover the rest.

Copayment

The predetermined, fixed amount you are required to pay for a covered service, like a doctor’s office visit or a trip to the ER. Copayments can be in effect before or after your deductible is paid, depending on your health insurance plan. This information is not shown on our health insurance verification form but you should be aware of what copayments you may be responsible for.

Coinsurance

A coinsurance fee is the percentage of the cost of a covered healthcare service that you will still be responsible to pay once your deductible has been met in full. For example, if you’ve paid your deductible, and the allowed amount for a doctor’s visit on your plan is $75, and your coinsurance is 20 percent, you will pay 20 percent of $75, or $15.

Out-of-Pocket Maximum

This is the maximum amount of money you will be required to pay for covered services within the plan year. This amount includes deductibles, copayments, and coinsurance fees: once you have reached our out-of-pocket max, your plan should pay 100 percent of any and all additional costs of in-network and/or covered health care costs.

Enrollment in a health insurance plan must be done either during the open enrollment period, usually held for a set amount of time once a year, or during a special enrollment period.
